There are more than 100 infectious diseases that are reportable to any given health jurisdiction. In most hospitals, case reports are done by filling out a paper form and faxing the same report to multiple Federal (CDC), State , and Local Public Health Agencies. The case reports are then re-entered into the Public Health Agencies system.
